LENS REPLACEMENT

Dr. Matthew Russell explains what life is like after lens replacement

LENS SURGERY EXPLAINED

What you need to know about lens surgery

Enjoying everyday life without relying on glasses or contact lenses is a goal for many people. In some cases, this cannot be achieved with laser eye procedures such as LASIK or PRK alone. This may be due to age-related changes in the eye, the strength of your prescription, or the overall health of your eyes.

If you wear reading glasses or varifocals, or if laser eye surgery is not suitable for you, lens surgery may be an option worth exploring. Lens surgery refers to procedures that correct vision by replacing or supplementing the eye’s natural lens.

There are two main approaches. One option is to replace the natural crystalline lens with an artificial intraocular lens implant (IOL). Another option is to place an implantable contact lens (ICL) inside the eye, in front of the natural lens.

Any surgical or invasive procedure carries risks. Before proceeding, you should seek a second opinion from an appropriately qualified health practitioner.

There are two types of lens surgery.

  1. Lens replacement removes your eye’s natural lens and replaces it with an artificial lens. This procedure improves the way light passes through the eye and focuses on the retina.
  2. Implantable Contact Lenses (ICLs) work just like removable contacts, except that they are surgically inserted into the eye.

Lens replacement surgery or refractive lens exchange (RLE) is virtually identical to cataract surgery, except the lens we replace is clear rather than cloudy.4Rcophth.ac.uk. 2021. [online] Available at: [Accessed 28 June 2021].

This procedure has a history that goes back to 1949 and is the most commonly performed elective surgery technique globally.5Davis, G., 2021. The Evolution of Cataract Surgery. [online] PubMed Central (PMC). Available at: <https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6139750/#:~:text=Cataract%20surgery%20is%20one%20of,treatments%20in%20all%20of%20medicine.> [Accessed 28 June 2021].

We use lens replacement to fix:

We can surgically insert contact lenses (ICLs) into the eye, where they cannot be seen or felt.

We use ICLs to fix:

Eye lens surgery may be considered for people who:

  • Are dependent on reading glasses, bifocals, or varifocals
  • Have prescriptions that fall outside the safe range for laser eye surgery
  • Have early lens changes or age-related focusing issues (presbyopia)
  • Have stable vision and healthy eyes overall

ICLs are more commonly considered for people with very high short‑sightedness or long‑sightedness who are not suitable for laser treatment. Lens replacement may be discussed with people whose vision changes are related to ageing or lens clarity.

A comprehensive eye assessment is required to determine whether lens surgery is appropriate. Not everyone will be suitable, and in some cases, no surgical treatment may be recommended.

Potential advantages may include:

  • Reduced reliance on glasses or contact lenses
  • Long‑term vision correction, as artificial lenses do not age
  • A wide range of lens options to suit different visual needs

Considerations and limitations include:

  • Lens surgery is an intraocular procedure and is more invasive than laser eye surgery
  • Recovery may take longer than surface laser procedures
  • Vision outcomes vary between individuals

Your surgeon will discuss expected outcomes based on your eye health, lifestyle, and visual needs.

All surgical procedures involve some degree of risk. With lens surgery, these may include:

  • Infection or inflammation inside the eye
  • Increased eye pressure
  • Retinal complications in rare cases
  • The need for further procedures

Serious complications are uncommon, but they can occur. A detailed discussion of risks, alternatives, and expected outcomes is an essential part of the consent process.
